Client Agreement Form

Policies & Terms   Heavens Gateway Ministries, Inc. (HGWM) has established policies to protect the organization, its partners, and donors. Please read carefully before signing.

By signing below, you agree to abide by the policies, regulations, and procedures of HGWM under the Good Samaritan Act.

1. Item Usage & Restrictions

• Items received from HGWM must not be traded, bartered, or sold.

• If a client or partner is found violating this policy, they will be removed from the program.

2. Food Expiration Disclaimer

• Some food items may have past the ‘Best By’ date.

• By signing this agreement, you acknowledge that you are consuming or using these products at your own risk.

• Clients waive the right to take legal action against HGWM or its donors for any side effects that may result from using these items.

3. Cost & Administrative Fee

• 90% of items distributed through HGWM are donated and free of charge.

• However, to support operational costs, clients who are financially able are encouraged to contribute a $37 administration fee.

• This contribution helps HGWM continue serving families in need and fulfilling its mission: “No family goes hungry.”
